The game begins with the dealer turning over a card to the player on his right.
This player must then guess whether the penis on the next card will be bigger or smaller.
If the player gets it wrong, they either drink a shot or drink for the number of seconds corresponding to the distance of their guess; or simply add a truth or dare element, take a photo or remove an item of clothing.
